Need Access To MyData?
Teachers do not need to submit a request for access unless SCHOOL-WIDE access is necessary. Both forms below must be submitted if requesting school-wide access.
If you are a teacher missing classroom access please send an email to firstname.lastname@example.org with your employee number and school name. We will contact you via email or phone once your access gets updated.
Certificated school office employees, only one form is required. Classified staff, both forms below are required.
Some users might also be able apply via EZ Access and submit an online application (only for school office personnel and local district). For District-wide access, please fax the MyData User ID Authorization form or send an email to email@example.com
