Notice Regarding Return to School
As we prepare for all our Early Childhood schools to reopen next week, I wanted to remind you that substitute teachers physically reporting to LAUSD campuses will need to have a negative Covid-19 Test prior to stepping onto a campus.

INFLUENZA VACCINATION DECLINATION FORM
Employees are responsible for maintaining their permit/credential. Proper certification must be on file. Failure to provide proof of renewal of your permit/credential with California Commission on Teacher Credentialing by the expiration date will result in your assignment being terminated at the close of the expiration day. This will preclude any further payment by the District for such assignment, and, as such will cancel all current medical, dental, optical, and insurance benefits.
All LAUSD employees must obtain an updated TB test every four years.
It is the teacher's responsibility to keep his/her CPR and First Aid cards up to date.
