LAUSD School-Based Health Centers and Services
At several LAUSD-operated School Based Health Centers, school physicians and/or nurse practitioners provide preventive health assessments and preventive health-related services to CHDP-eligible children and teens.The following services are available free of charge to eligible LAUSD students and their siblings ages 3-18 who do not have health insurance or access to a regular doctor:- Complete Physical Examinations

Prepare For Your Visit
The following tips will help you get ready for your visit:• Children must be accompanied by a parent or guardian at all times.
• If your child is getting an immunization, bring his or her yellow Immunization Card so it can
be updated.
• At the time of the visit, consent from the parent or guardian is a requirement to receive
CHDP-related LAUSD school-based health clinic services.
• Be on time if you have an appointment or it may be rescheduled.
• If you need to cancel an appointment, do so at least 24 hours in advance.
• Be prepared to wait; bring a book or other activity to keep you busy during the wait.
• Eat something before you arrive. Eating and drinking are not allowed in the clinic.

LAUSD School-Based Health Centers and Services
Serving Eligible Students and their Siblings ages 1-18 & Special Education Students thru age 22. School-Based clinics are staffed by L.A. Unified school physicians and nurse practitioners who see only students and their siblings. Families can receive help with enrolling their children into Medi-Cal to receive services, but these clinics are “assessment only.” In other words, they perform physicals and give immunizations but do not provide continuous care for patients with chronic conditions. Reproductive health care is available at locations marked with (*) (STD testing, birth control and pregnancy testing). Vision exams are also available at locations marked with (^). School Physicians
Health services are available District-wide on a limited basis upon request. Below is a general list of services facilitated by LAUSD school physicians located in each Local District area.Medical Exams and Appraisal for:- Students with special health needs in special education instruction.
- Pre-participation sports clearance for interscholastic sports and Special Olympics.
- Infants and toddlers with special needs may be eligible for early intervention services.
- Children who need a school entry exam and do not have access to a regular medical provider.
Related Services:- Confer with school personnel, students' physicians, and parents regarding classroom placement and/or management of various health problems.
- Diastat Training
- Occupational Therapy and Physical Therapy Related Services
- Issue medical permits/exclusions/exemptions/sports participation waivers when necessary.
- Provide and/or arrange emergency care of students ill or injured at school.
- Provide medical consultation to local district personnel, special education staff, and nurse in the assigned districts.
- Provide clinical supervision to nurse practitioners and medical assistants.
Requests for Services:Requests for services are usually initiated and/or coordinated through the onsite school nurse, appropriate designated school personnel, and/or District Nursing. Generally, requests are screened, assessed, and approved by the Local District attending school physician. -
