Obtaining Evidence of Self-Insurance

Lessors, vendors or contractors requiring proof of the District’s self-insurance programs can obtain documentation by faxing a REQUEST FOR PROOF OF SELF-INSURANCE to 213-241-8956.

Include your name, the facility address, reference the contract or lease agreement including dates or use or contract period along with the name of the school or District Department, and your District contact.
